Discussion was held on the topics of current enrollment and current staffing.

Comments:
The following inspection took place between the hours of 11 am through 2 pm. Today, I met with the Staff of the Goddard School of Leesburg located in Leesburg, Virginia. At the start of this unannounced monitoring inspection there were a total of 84 children with11 classroom staff; good ratios in all 8 rooms. A total of 5 children?s files and 7 new staff files were reviewed. The children were engaged in the following: Freeplay, toys, story time, diaper changing, bathroom breaks, hand washing, lunch, nap time, and other organized activities. Standard #: 22VAC40-185-70-A
Description: Based on review, 6 of 7 staff files reviewed indicated that not all of the required documentation was obtained. Evidence - 1. On the date of inspection (03/27/2015), 0 of 2 references were on file for: Staff #1, Staff #2, Staff #3, Staff #5, and Staff #7. Only 1 of 2 references was on file for Staff #4. 2. The 7 staff files reviewed were for those new employees who had start dates after 09/01/2014.

Plan of Correction: All missing information will be obtained and placed on file.

Standard #: 22VAC40-185-270-A
Description: Based on observation, two separate pieces of equipment of the center, inside were not maintained in a safe and operable condition. Evidence - 1. On the date of inspection (03/27/2015), a floor mat within the Infant A room showed signs of wear. The corners of the mat were frayed and were accessible to the children in care. 2. A sink within the Preschool (3 - 4's) room was leaking. A small plastic contained was placed underneath in order to contain all drips.

Plan of Correction: The administration was contacted and has discussed the issue of replacing the mat. Maintenance has been contacted in order to fix the sink.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances (such as cleaning materials) were not kept in a locked place using a safe locking method that prevents access by children. Evidence - 1. On the date of inspection (03/27/2015), a spray bottle containing a mix of bleach and water was observed in a spray bottle being stored on the top of a classroom cabinet, not in use. 2. A staff person within the room at the time who was leading an activity on the rug confirmed that the bottle contained bleach and water.

Plan of Correction: All cleaning supplies or any sanitizer will be locked up.

Standard #: 22VAC40-185-570-E
Description: Based on observation, prepared infant formula that was being refrigerated were not labeled with today's date. Evidence - 1. On the date of inspection (03/27/2015), two bottles within the Infant B room refrigerator were not labeled with today's date. 2. The child's name did appear on both bottles. 3. The staff within the room confirmed this observation on the date of inspection.

Plan of Correction: The date will be added to both bottles. Staff will monitor in order to ensure all future compliance.
