Comments:
An unannounced monitoring inspection was conducted today, April 13, 2015. Inspection began at 2:20 pm and ended at 4:00 pm. A walk through of the building was conducted. A violation notice was completed regarding the violations found during the inspection. A review of the staff records was conducted on 4/20/15 at the clubs main office. There were 57 children in care with five staff and one maintenance staff present during the inspection. Should you have any questions please do not hesitate to call.

Violations:
Standard #: 22VAC40-185-160-C
Description: Based on review of staff records it was found that TB tests are not being resubmitted every two years. Evidence: 1. Staff #5 has a TB test that expired 7/23/14 and staff #6 has a TB test that expired 1/22/15.

Plan of Correction: The staff will schedule appointements with the local health department to have their TB tests/screens updated. Files will be monitored to ensure that all required information is updated.

Standard #: 22VAC40-185-280-B
Description: Based on observation of the center during the inspection on 4/13/15 it was found that hazardous substances are not being locked. Evidence: 1. Located in the cabinet above the sink was a bottle of Calamine lotion. 2. Located under the sink, which had one door missing, was a bottle of Drano-max gel; Proforce-oven, grill, fryer cleaner; box of Cascade; Finish dish detergent. 3. All of these items are unlocked and the kitchen door was opened which made this area accessible to the children.

Plan of Correction: The hazardous substances will be placed in a locked cabinet. Staff will monitor these areas to ensure that all hazardous substances are kept locked.

Standard #: 22VAC40-185-280-G
Description: Based on observation of the center during the inspection on 4/13/15 substitute containers are not labeled. Evidence: 1. Located under the kitchen sink was a spray bottle with blue substance inside. Located above the microwave was a spray bottle with a liquid substance inside. 2. Both spray bottles were not labeled indicating what they contained.

Plan of Correction: Any container that has a cleaning substance or hazardous substance inside will be clearly marked and locked when not in use.
