Technical Assistance:
1. Written parent permission, in addition to a developmental assessment, is needed prior to moving a child to a different age group, regardless of whether it is to a higher or lower age grouping.

Comments:
Conducted an investigation into allegations of ratio violations in the 2 year-old group on 4/2/15. Interviews conducted and records reviewed. There was a preponderance of evidence to support the allegation and therefore this complaint is deemed to be valid/founded. An additional violation that was not reported was found and cited.

Description: Based on records reviewed, it was determined that a 1:8 staff to child ratio for the 2 year-old group was not maintained on 4/2/15. Evidence: 1. According to the Child Supervision Record (CSR), at 11:00am the ratio was 1 staff person with 9 children. 2. According to the Child Supervision Record (CSR), at 11:30am the ratio was 1 staff person with 10 children.

Plan of Correction: Teachers were re-trained on ratios and reminded to always inform management when over-ratio. Management has improved ratio tracking by using a ratio count sheet daily.

Description: Based on records reviewed, it was determined that a child was being moved to a different age group without written parent permission, and repeatedly disrupted the child's schedule by moving the child between three (3) age groups in one day. Evidence: 1. Child #1 - Is a 2 year-old, and is currently assigned to the Toddler group. 2. The written assessment completed by the Lead Teacher and signed by the Director on 3/31/15, indicated the most appropriate group for the child developmentally was the Toddler group (ages 16 - 24 months); however, there was no written parental permission on-file (Parent Transition Letter). 3. According to the CSR on 4/2/15, the child was moved from the Toddler group to the 2's group from 3:30-5:10pm; and at 5:10pm from the 2's group to the Infant group for the remainder of the day.

Plan of Correction: Teachers were retrained on transitions and reminded that children should be moved the least amount possible. Management is working on a parent sign off to keep children in a classroom longer (i.e. a 2 yr. old staying in toddlers) if it is in the best interest of the child.
