Unannounced inspections were conducted on March 30, 2015 and April 16, 2015 in response to a complaint called in to the licensing office concerning infant care. Based on observation, record review and staff interviews, the complaint is valid. A violation notice was issued. Thank you for your cooperation. Please contact me if I may be of service.

Description: Based on observation, the facility did not maintain all areas in a clean and operable condition. Evidence: 1. The rug in the infant area is soiled. There are several areas where there is lint, dried particles and stains.

Plan of Correction: The director reported that the rug will be cleaned by a professional rug shampooing company.

Description: The facility did not place all infants on their backs to sleep. Evidence: 1. In an interview, staff #1 reported that they sometimes place children on their stomachs to sleep.

Plan of Correction: The director retrained all staff to insure that the caregivers place infants on their backs to sleep.

Description: The facility did not label all cribs. Evidence 1. On March 30 and on April 16, 2015 during the tour of the infant room, the LI observed that the facility did not label cribs with children's names.

Plan of Correction: The director will remind staff to label all cribs.
