Comments:
A complaint investigation was conducted at your facility on 04/27/2015 regarding licensing standard 22 VAC 40-185-(5)-350 E. Staff to children ratio requirements. Upon the completion of the investigation and based upon the preponderance of evidence, the allegation in the complaint, regarding licensing standard 22 VAC 40-185-(5) 350 E. Staff to children ratio requirements could not be substantiated and the complaint for this standard was determined not valid. Other areas inspected today were playground and facility space, staff records, children's records, attendance documentation, and diapering areas. There were 27 children with 8 staff members at the time of the inspection and all rooms were in ratio compliance.

Description: Based on observation the center did not have protective covers on all electrical outlets. Evidence: 5 outlet caps were missing on the power strip in the infant room near the changing area.

Plan of Correction: Outlet covers were placed in the power strip during the inspection as required.
