Technical Assistance:
Discussion was held on the topics of current enrollment and current staffing. Discussion of the daily logs that are computerized within the Infant Room. While tummy time is described, and often pictures are taken of its use, further documentation with regards to actual time and duration needs to be noted.

Comments:
The following inspection took place between the hours of 11:30 am through 2:30 pm. Today, I met with the Staff of the Kiddie Academy located in Centreville, Virginia. At the start of this unannounced monitoring inspection there were a total of 110 children with 18 staff; good ratios in all 6 rooms. A total of 4 children?s files and 14 new staff files were reviewed. The children were engaged in the following: diaper changing, hand washing, bathroom breaks, learning, games, toy play, art, infant feeding, song, music, lunch (hot dogs on bun, green beans, apple, served with milk), nap time, and other organized activities. Standard #: 22VAC40-185-60-A
Description: Based on review, 2 of 4 children's files reviewed indicated that not all of the required information was on file. Evidence - 1. On the date of inspection (04/29/2015), the following information was not available for review: parent work information (for Child #1), and the complete information for 2 emergency contacts (for Child #1, and Child #2). 2. This was confirmed on the date of inspection by the Center Director.

Plan of Correction: All missing information will be obtained and placed on file.

Standard #: 22VAC40-185-70-A
Description: Based on review, 2 of 14 new staff files reviewed indicated that not all of the required information/documentation was on file. Evidence - 1. On the date of inspection (04/29/2015), the following documentation was not available for review: educational documentation for: Staff #3, and Staff #6. 2. This observation was confirmed on the date of inspection by the Center Director.

Plan of Correction: All missing information will be obtained and placed on file.

Standard #: 22VAC40-185-270-A
Description: Based on observation, two pieces of equipment inside the center were not maintained in a safe condition. Evidence - 1. On the date of inspection (04/29/2015), two pink chairs were observed with rips in their seats. 2. The chairs were not in use and appeared to be "game chairs" that sit on the floor.

Plan of Correction: The chairs will either be discarded, repaired, or replaced.

Standard #: 22VAC40-185-510-J
Description: Based on observation, one medication was observed not in a locked place using a safe locking method that prevents access by children. Evidence - 1. On the date of inspection (04/29/2015), an Epi-pen was observed in an unlocked upper cabinet within the 2's room. 2. This observation was confirmed by classroom staff on the date of inspection.

Plan of Correction: Corrected on the date of inspection. The medication was moved to a locked area.
