Comments:
Arrived 9:25am Departed 1: 35 pm An unannounced monitoring inspection was completed today. Four staff records and children's records were reviewed. Four infants were present with two staff. Four toddlers were present with one staff member. Thirteen twos were present with two staff. Thirteen threes and fours were present with two staff. The children were playing in centers, having group time, taking a nature walk, playing outdoors, eating lunch and resting. Thank you for your cooperation. Please contact me if I may be of service.

Standard #: 22VAC40-185-270-A
Description: Based on a tour of the preschool room, the facility did not secure furniture all areas. Evidence: 1. The LI observed a large red wooden shelf (6 1/2 ft L X 42 inches high X 13 inches deep) in the home living area. 2. The LI observed the shelf rocking when a child stepped on it. 3. This shelf is heavy and presents a tipping hazard.

Description: Based on a tour of the child care building and the Club 88 building, the facility did not secure all hazardous materials. 1. There is a plastic container of Clorox Wipes on a table in Club 88. 2. The cabinet under the sink in the kitchen of Club 88 the padlock is hanging in the hasp but is not locked and contains two containers of Clorox Wipes, a gallon of bleach, eight cans of spray paint, two containers of liquid Goo Off, one bag of 4 in 1 Crystal Burst detergent packets, liquid Lysol multi surface cleaner, one container of Clorox bowl cleaner and one container of Blue Wolf heavy duty degreaser. In the preschool child care building: 3. In the women's bathroom, under the sink in the infant area the LI observed one container of Windex, one container of liquid spot remover, two aerosol cans of air freshener and hand sanitizer. The cabinet is not locked. 4. In the men's bathroom, under the sink in the infant area the LI observed that there is a lock on the cabinet door but it is not locked. The cabinet contains liquid purple degreaser and liquid floor stripper. 5. In the twos' diapering counter top, in the lower cabinets, there is no lock. This cabinet contains: three aerosol cans of air freshener and hand sanitizer, a plastic container of Clorox wipes, liquid Lysol multi-purpose cleaner, one bottle of Clorox toilet bowl cleaner, one bottle of Windex and two cans of Great Value disinfectant.

Description: Based on a tour of the playground the facility did not maintain a six foot perimeter around climbing equipment. Evidence: 1. The LI observed that there is 46 inches between the end of the new plastic climber slide and the yellow slide on the wooden climber.

Description: Based on observation in the infant room, the facility did not provide continuous sight and sound supervision of all children. Evidence: 1. An infant was playing with soft toys and blocks toys on the carpet near a low shelf. 2. For five minutes the child played and no staff member checked on or looked at the child. 3. Staff #1 was feeding a child and staff #5 was changing a child's diaper.

Description: Based on a tour of the twos' diapering area the facility did not maintain a non-hand-operated can for diaper disposal. Evidence: 1. The diaper trash can must be operated by hand as the foot pedal is missing.
