Comments:
An unannounced monitoring inspection was conducted on 5/12/15 from 3:15pm - 4:30pm. At the time of entrance there were 45 school age children in care with nine staff. Children were observed doing homework, playing outside and eating snack. Records were reviewed for five children and for three staff. Medication, emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice, and discussed during the exit interview. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and returned it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Violations:
Standard #: 22VAC40-185-550-D
Description: Based on a review of the emergency drill logs, the facility did not ensure that a monthly practice evacuation drill for the most likely to occur scenarios was completed. Evidence: 1. The emergency drill log did not have an entry for February 2015. There was a practice evacuation drill completed for January 2015, March 2015, and April 2015. 2. Staff #1 (Program Director) confirmed that no practice evacuation drill was completed during February 2015 due to weather.

Plan of Correction: We did not have a February fire drill because of numerous days missed due to snow and poor weather. We will plan to have drills closer to the beginning of the month during the winter to avoid this problem in the future.
