63.2 General Provisions.

Comments:
An unannounced monitoring inspection was conducted on 5/13/15 between 1:30 and 3:00. The inspection was conducted with the director. The inspector spoke with staff and children. The following staff/child ratios were observed: 2, 3 & 4?s - one staff with eight children napping 3?s - one staff with 9 children napping 4?s - one staff with 10 children The children were napping when the inspector arrive. Later they were observed getting up from nap and using the restrooms to wash up for snack. Snack this afternoon was donuts and milk. Lunch today was chicken nuggets, green beans, corn, roll and milk. Health inspection ? 10/31/14 Fire inspection ? 6/3/14 You will need to contact the Henrico Fire Marshal to schedule an inspection next month. Last fire drill 4/23/15 and the last shelter-in-place drill was conducted on 2/5/14 All required documents were posted including license, licensing notices, menu, and evacuation information.

Violations:
Standard #: 22VAC40-185-280-B
Description: The center failed to ensure that all cleaning materials were kept in a locked place. Evidence: The inspector observed a spray can of Febreze sitting on the high file cabinet in the office. The office was not locked.

Plan of Correction: Put it with cleaning supplies

Standard #: 22VAC40-185-290-3
Description: The center failed to ensure that all electric outlets had protective covers. Evidence: The inspector observed to power strips in the office. One under the knee space of the desk and one by the A/C unit. There were a few outlets on each strip that did not have anything plugged into it. Some of the unused outlets had covers, but there were a few in each power strip without covers.

Plan of Correction: Covers were there just need to be put in.

Standard #: 22VAC40-185-320-B
Description: The center failed to ensure that the water temperature at the bathroom sink used by the children did not exceed 120 degrees. Evidence: The inspector used a thermometer to test the water temperature in the restroom off of the room used by the two year old children. This is the restroom used for diapering. The water temperature tested at a high of 130.1 degrees, which is over 120 degrees.

Plan of Correction: Have plumber fix it again.

Standard #: 22VAC40-185-440-E
Description: The center failed to maintain the required space around the occupied mats used for nap time. There needs to be at least 12 inches of space around each occupied mat. Evidence: The inspector observed the children on their mats at nap time. There were nine children napping in the 3's (front) classroom and 10 children napping in the 4's (back) classroom. At least four mats did not have 12 inches of space between the mats in the front room and most of the mats in the back room were less than 12 inches apart. The room measurements allow for six children in the front room and five children in the back room. There were too many children napping in both rooms for the size of the rooms.

Plan of Correction: Make sure 12 inches each day starting 5/14/15.

Standard #: 22VAC40-185-500-B
Description: The center has failed to ensure that the diapering surface is nonabsorbent. A changing table has been obtained for the children who are younger that three years. There is a mat being used as the diapering surface. Evidence: The inspector observed the mat placed on the changing table. There is a torn place on the mat. The mat is no longer nonabsorbent and can not be properly sanitized.

Plan of Correction: Get bath mat.

Standard #: 22VAC40-185-550-D
Description: The center has failed to conduct a minimum of two shelter-in-place years this last year. Evidence: The inspector reviewed the evacuation and shelter-in-place log for the past year. The center's last documented shelter-in-place drill was 2/5/14. There is no documentation of any drills during the last 15 months.

Plan of Correction: Do drill today
