Comments:
The licensing inspector conducted an unannounced focus inspection May 13, 2015 from approximately 10:00am to 1:40pm. The following ratios were observed with a total of 50 children present during the inspection: Infants - 11 children with 2 staff (see violation notice) Ones - 4 children with 1 staff Ones - 3 children with 1 staff Twos - 10 children with 2 staff Threes - 5 children with 1 staff Threes - 7 children with 1 staff Fours - 10 children with 2 staff School age children were not present during the inspection Children and staff were observed participating in a variety of activities during the inspection. Infants were observed sleeping in their cribs, being rocked and eating. The Ones and Twos classrooms were observed playing outside. The Threes classrooms were observed eating lunch, taking potty breaks and looking at books. The Fours were observed eating lunch and playing outside. Naptime was also observed. During the inspection, five children's records and four employees' records were reviewed. Violations:
Standard #: 22VAC40-185-160-C
Description: Based on review of four staff records, one record did not contain an updated negative tuberculosis screening. Evidence: During the inspection, the licensing inspector observed the record for Staff #1 (hire date 06/20/11) contained a tuberculosis screening dated 10/19/12. Negative tuberculosis screenings are required to be updated at least every two years from the initial screening.

Plan of Correction: Per owner - the staff member has an appointment to get an updated T.B. test on 05/15/15.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the center did not maintain all areas of the center in a safe and operating condition. Evidence: During the inspection, the licensing inspector observed a tan cloth barrier placed underneath the resilient surfacing had worked its way to the surface and was exposed in several areas on the Toddler playground. The exposed cloth creates a tripping hazard.

Plan of Correction: Per Owner - The tan cloth barrier will be removed from the playground this weekend.

Standard #: 22VAC40-185-350-E-1
Description: Based on observation and interviews, it was determined the center did not ensure that whenever children from birth to 16 months were in care that there was one staff member for every four children present. Evidence: At approximately 11:15am, the licensing inspector observed 11 Infants present in the Infant Room being supervised by two staff members. The licensing inspector inquired why the required ratio of one staff member for four infants present was not being followed. A member of management stated the third staff member went to lunch. A member of management stated they thought if all the infants were sleeping the ratio was allowed to be doubled.

Plan of Correction: Per owner - Corrected during the inspection. We believed the Infant ratio was could be doubled if all of the children were sleeping. An additional staff member was added to the classroom when the required ratio was pointed out to staff members.

Standard #: 22VAC40-185-560-F-4
Description: Based on observation and interviews, the center did not ensure foods that are potential choking hazards are not served to children three years of age or younger. Evidence: During the inspection, the licensing inspector observed three year old children eating whole size corn dogs on a stick. A staff member stated children in the Twos classroom also eat the corn dog off of a stick. Hot dogs that are not cut in half are potential choking hazards.

Plan of Correction: Per owner - We will make sure the corn dogs are cut up in the future.
