Comments:
An unannounced monitoring inspection was conducted on May 4, 2015 from approximately 11:15 AM to 3:00 PM. Seven staff were present with twenty- seven children. Children were observed during music and movement, lunch time, nap time, hand washing and diaper changing procedures. Three children records, two staff records and two injury reports were reviewed on this date. Licensing Inspector reviewed supervision, activities, equipment, diapering procedures, hand washing procedures, nutrition, special feeding, injury report documentation, children's records, emergency evacuation and shelter-in-place drill documentation, health inspection report, emergency supplies and required positing. Playground was inspected . The provider's response for the "plan of correction" was not received as of May 18, 2015 and will not appear on this Violation Notice.

Description: Based on 2 of 2 record reviews and staff interviews, the licensee did not ensure each staff record contained all required documentation. Evidence: During the inspection conducted on May 4, 2015, the records for Staff #1 and Staff #2 did not include documentation of job title and written information of qualification. Staff #3 confirmed there was no documentation of job title or information of staff qualifications.

Description: Based on 1 of 2 records reviewed, the licensee did not ensure staff met required annual training hours. Evidence: During the inspection conducted on May 4, 2015, the record for Staff #2 had 10 hours of annual training documented for 2014.

Description: Based on record review and interview, the licensee did not ensure there was at least one staff member on duty who has obtained within the last three years instruction in performing the daily health observation of children. Evidence: During the inspection conducted on May 4, 2015, there was no documentation available indicating there was a staff member on site who obtained daily health observation in the last three years. Staff #3 confirmed there was no staff at the center with daily health observation training.

Description: Based on observation, the licensee did not ensure hazardous chemicals are kept locked. Evidence: On May 4, 2015, Licensing Inspector observed the following : -In the toddler's classroom, there was bleach in water in the unlocked cabinet above the sink next to the changing table. There was a padlock but it was not locked. -In the two year old's classroom, there was hand sanitizer on the desk in the classroom. The hand sanitizer was labled "Keep out of reach of children" and "caution ." The handsanitizer was on the desk when the children were coming in from outside time. -In the classroom where the three and four year old's eat lunch there was a unlocked closet with Clorox wipes and Clorox Spray Anywhere on a shelf. The items were labeled with "Caution" and "Keep out of reach of children."

Description: Based on observation, licensee did not ensure there is at least 12" of space between occupied cots during nap time. Evidence: On May 4, 2015, Licensing Inspector observed in the classroom 10 children, ages 3 and 4 years old, napping in a classroom. The cots were approximately 6 inches between each cot. Staff verified that she was was not sure of the spacing needed between each cot.

Description: Based on observation, facility did not ensure staff wash their hands after diapering a child. Evidence: On May 4, 2015, Staff #2 in the infant classroom was observed changing child #1's diaper. Staff did not wash her hands after changing the child's diaper, then continued to get a bottle for child#1. Staff #4 completed cleaning the changing area and disposing the diaper for child #1. Staff #4 did not wash hands after cleaning the changing area and continue to pick up a child.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the licensee did not ensure requirements are met when children's diapers are changed. Evidence: On May 4, 2015, Licensing Inspector observed the following: 1. Staff #2 in the infant class change child #1 on a pink cloth on top of the diapering pad. The pink cloth cause the surface to become absorbent. 2. Staff #4 in the infant classroom sanitize the diapering surface area after Staff #2 diapered child #1. Staff #4 disposed the diaper, the diapering pad was cleaned with soap and water, sprayed the bleach/water solution on the diapering pad and then immediately wiped the surface with a paper towel and let it air dry. 3. In the infant room, the foot operated diaper pale lid would not open. Staff in the infant room verified the diapering pale was not working.

Description: Based on 1 of 2 injury reports reviewed, the licensee did not ensure there was written record of the date and time parents were notified and how parents were notified. Evidence: During the inspection conducted on May 4, 2015, injury report for child #4 the date and time parent's were notified of the injury and how the parents were notified was not documented.

Description: Based on observation, licensee did not ensure infant bottles are labeled with name and date. Evidence: On May 4, 2015, Licensing Inspector observed a green bag labeled with child's name on the outside of the bag, inside the bag there was a pre-made bottle and pre-made formula not labeled with the child's name and date .
