Comments:
An unannounced monitoring inspection was conducted on May 8, 2015 at 9:15 a. m. and concluded at 11:30 a. m. The inspector met with the staff and some but not all Child Day Center standards were reviewed. The following staff-to-child ratios and activities were observed. Infants: There were eight infants with two staff. The staff was observed holding and feeding the infants. Several infants were having free play. Toddlers: There were five toddlers with one staff. The toddlers were observed having free play. Two Years: There were eight children with one staff. The children were observed having circle time which included singing "The days of the week", going over the calendar, weather and talking about feelings. Preschool (Three Years) There were nine children with one staff. The children were engaged in a variety of activities which included: car track center, dinosaurs at the table and Home Living. Pre-K (Four-Five Years): There were 19 children with two staff. The children were engaged in a variety of interest centers which included a garden center, blocks, using scissors and cutting picture out of magazines, puzzles, Legos, and making Mother Day Cards. Three children's records were reviewed. One staff record were reviewed. Two classroom observations were conducted. Conversations were held with nine staff and five children. No children were on medications today, two staff currently trained in MAT (medication administration trained). The last monthly evacuation drill was documented on 4/22/15. Two shelter-in-place drills were conducted in 2014.

Standard #: 22VAC40-185-280-B
Description: Based on observation the center did not have hazardous substances stored in a locked place. Evidence: On 5/8/15, the inspector observed one can of Lysol, one bottle of disinfectant, and one bottle of bathroom cleaner being stored in a cabinet inaccessible to children (but unlocked) in the two year classroom bathroom. The lock was broken on the cabinet that was storing these cleaning supplies.

Plan of Correction: A work order is already in place to replace the lock on the cabinet.
