Technical Assistance:
Today we discussed documentation of qualifications for program leaders. Copies of degree and or transcripts are acceptable. Remember to document 2 shelter-in-place drills for the 2014 - 2015 school year.

Comments:
An unannounced monitoring inspection of the facility and records was conducted from 8:15am ? 11:1apm. There were 20 children in care with 4 staff members. Interviews were conducted with children and staff present. Six children?s records and one staff record were reviewed. Children were observed in teacher and self-directed play activities, transitions and chapel. Any repeat violations and or areas of non-compliance are identified on the violation notice.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation the center failed to maintain equipment in safe condition. Evidence: 1 - There are areas of peeling paint on the the metal climbing structure and the climb and slide equipment on the preschool playground. 2 - The peeling paint is within reach of children when playing on the equipment.

Plan of Correction: Center management will have the peeling paint removed and equipment painted.

Standard #: 22VAC40-185-330-B
Description: Based on observation the center failed to ensure that use zones are maintained as required. Evidence: There is a yellow metal climber on the preschool playground that is 5.5 feet from a chain link fence. The equipment requires a 6 foot use zone on all sides.

Plan of Correction: Center management will have the equipment moved so that there is no less than a 6 foot use zone on all sides of the yellow climbing equipment.

Standard #: 22VAC40-185-550-D
Description: Based on interview and record review the center failed to hold monthly practice evacuation drills. Evidence: 1 - The documentation for fire drills, provided to the inspector, during the inspection, does not include documentation of practice drills completed during the months of December, 2014 and January 2015. 2 - Staff 1 confirmed that drills were not completed during December and January.

Plan of Correction: Center managements notes that due to weather related school closures the drills were missed. Going forward the center will ensure that drills are completed on a monthly basis as required.

Standard #: 22VAC40-185-560-G
Description: Based on observation the center failed to ensure that when food is brought from home the food container shall clearly dated. Evidence: There are four lunch boxes in classroom one that are not labeled with the date.

Plan of Correction: Center management will remind and retrain staff that each lunch box must be labeled with the current date each day. Center management notes that there is less than a week remaining in the school year. Staff will again be reminded in September.
