Comments:
An unannounced monitoring inspection was conducted from 9:30 a.m. - 1:25 p.m. There were fifty-two children in care with nine teachers working directly with them. There were three other personnel available to assist as needed. Four children's records were reviewed. One substitute teacher's record was reviewed and one other teacher's record was reviewed. The younger children were observed being fed, burped, sleeping, playing, being comforted by the teacher, watching the dangling toys on the activity mat, crawling and listening to piano music. The older children were observed preparing for lunch, practicing good manners, waiting patiently as the food was being served, working with the colors green and yellow, mixing different colors to make new colors, looking at and reading books, playing at various learning centers, enjoying the ocean life sensory table and talking with one another and laughing. The facility has established a teacher's resource nook and a children's library in coordination with the Regional Library. Areas of non-compliance are identified on the violation notice. Standard #: 22VAC40-185-440-B
Description: Based on inspection there was one crib with a child sleeping in it that was not labeled identifying for use by this specific child. Evidence: 1. On 06/03/2015 the licensing inspector could not see the child's name on the crib identifying that the crib was for his use. 2. On 06/03/2015 staff #1 confirmed that there did not appear to be a name identifying that the crib was specifically for the child's use.

Plan of Correction: The name was found on the backside of the crib which was not visible. We made another name tag for the crib immediately. We will continue monitoring the cribs to make sure the name of the child is clearly visible. The cribs had been washed down and sanitized so this probably happened at that time.
