Comments:
The licensing inspector conducted an unannounced monitoring inspection from 10:15am - 1:00pm. There were twenty six children in care: four infants in the young infant room (ages 4 - 6 months) with one staff, four infants in the older infant room (ages 10 - 14 months) with one staff, ten children ages 20 months - 3 years with two staff and eight preschoolers with one staff. Two additional staff members were also present. The children were observed playing on the playground, during lunch and preparing for naps. Three staff and three children's records were reviewed. Areas of non-compliance are identified on the violation notice. If you have any questions, please contact your licensing inspector at (276) 676-5634. Thank you for your cooperation.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record reviews, the center failed to obtain all required information for the children's records. Evidence: 1. The inspector reviewed three children's records. One of three records reviewed (child record #2) did not address any chronic physical problems and one of three records reviewed (child record #3) did not address allergies and actions to take in an emergency situation nor chronic physical problems.

Plan of Correction: The director will request this information from the children's parents. The director will review the children's records quarterly to ensure all information is complete.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the center failed to maintain the facility in a safe condition. Evidence: 1. The inspector observed the following: in the older infant room, there is a crib that has peeling paint; on the gate leading to the toddler outside play area there is a loose screw and peeling paint.

Plan of Correction: This crib has been removed from the infant room. The screw has been removed from the gate. The gate will be painted. The director will monitor the facility for unsafe conditions.

Standard #: 22VAC40-185-420-E-1
Description: Based on interview, the center failed to maintain a daily record for all infants in care. Evidence: 1. The inspector asked for the daily records for the infants in the older infant room (ages 10 months - 14 months). The director said they were not keeping daily records for the infants in this classroom.

Plan of Correction: The center will begin keeping daily records for all infants until they are 16 months old. The director will monitor to ensure the records are completed daily.
