Description: Based on record reviews, the center failed to obtain reference checks on staff before employment began. Evidence: 1. The inspector reviewed two staff records. One of two records reviewed (staff record #1) did not contain any reference checks and staff #1's date of employment was 8/22/14. One of two records reviewed (staff record #2) contained only one reference check and staff #2's date of employment was 6/16/15.

Plan of Correction: Human resource staff attempted to contact the references that were listed for staff #1, but were unable to make contact with the references. Human resource staff will continue to try to make contact with staff #1's references and once information is obtained will record the results of the reference checks in staff #1's record. Human resource staff will obtain an additional reference check for staff #2. Staff records will be reviewed quarterly to ensure all information is complete.
