A work order was placed during the inspection. 7/27/15

Comments:
An unannounced monitoring inspection was conducted today by the licensing inspector. The center Director was present part of the time and the Assistant Director was present during the entire inspection. Some but not all standards were checked today and observations were done with age groups. Medication and first aid supplies were checked and lunch was also observed today.
Children were also observed on the playground. The facility participates in the USDA Food Program.

staff/child ratios observed:
toddlers 3:6 Staff were blowing bubbles while the children colored a bright yellow sun on a paper plate. The children looked at books, staff sat on the floor and read them a story. Some children played with puzzles. Diapering was observed in this classroom.

infants: 3:9 Some of the children were sleeping while the rest of the children were on the floor playing with toys, interacting with staff, crawling and walking around. One staff was doing fingerpaint with the children individually.

3's 2:13 The children were playing in learning centers throughout the classroom. They were playing with legos, blocks, using the water table to see what items could float, playing doctor/dramatic play, and making paper bag puppets.

4's 1:10 They were having water play outside then played hot potato on the playground (poured in place surface).

2's 2:12 Children were playing on the outdoor equipment then returned inside to have lunch.

Description: Based on observation, the facility did not maintain an outside area of the building in a safe condition. Evidence: On the 2 year old playground, to the right of the storage closet door, there is an area at the bottom of the siding where the wood has deteriorated creating an area approximately 6 inches wide with wood missing and a splintery area accessible.

Plan of Correction: A work order was placed during the inspection. 7/27/15
