An unannounced inspection was conducted at the facility on July 15, 2015 from approximately 9:00 am - 11:30 am. Upon entrance there were 48 children and 7 staff . The Licensing Inspector observed children playing with manipulatives, line dancing, station activities and preparing for a field trip . The Licensing Inspector reviewed 4 children records and 2 staff records. Medication is not given at the center. The playground was inspected. The exit interview was conducted with Staff #1 and director.
Standard #: 22VAC40-185-160-C Description: Based on 1 of 2 staff records reviewed, the licensee did not ensure staff had documentation of tuberculosis screening within the past 2 years. Evidence: During the inspection conducted on July 15, 2015, staff #1's last T.B. test was completed on July 10, 2013, which is more than 2 years old. Plan of Correction: T.B. is scheduled Director will make sure appoitment is followed through.
Standard #: 22VAC40-185-270-A Description: Based on observation, the licensee did not ensure that all areas and equipment of the center, inside, were maintained in a clean, safe and operable condition. Evidence: During the inspection conducted on July 15, 2015, the licensing inspector observed in the Pre-K classroom with multiple areas of peeling paint on the wall. Plan of Correction: Director had already scheduled painting in the month of August classroom needing paint or touch up will be done.
Standard #: 22VAC40-185-280-B Description: Based on observation, the licensee did not ensure all hazardous substances were kept in a locked place. Evidence: During the inspection conducted on July 15, 2015 , the licensing inspector observed in a unlocked staff bathroom the following hazardous substances3 cans of aerosol spray, hand sanitizer and lotion. The hazardous substances were labeled "Keep out of reach of children", "flammable"and "Warning." Plan of Correction: Our staff bathroom always has Lysol and aerosol sprays and lotions. In the future the director will make sure these items are in a locked to container.
Standard #: 22VAC40-185-290-3 Description: Based on observation, the licensee did not ensure electrical outlets had protective covers. Evidence: During the inspection on July 15, 2015, the licensing inspector observed in the infant room a power strip attached to wall with 3 electrical outlets that did not have protective covers . Plan of Correction: The electrical covers were removed from the outlets while cleaning. Power strip is located high on the wall . Director will make sure they will replaced and stay.
Standard #: 22VAC40-185-580-C Description: Based on observation and interviews, the licensee did not ensure the center had a prepared document containing local emergency contact information, potential shelters, hospitals, evacuation routes, etc., that pertain to each site frequently visited or routes frequently driven by center staff for center business (such as field trips, pickup/drop off of children to or from schools, etc.). This document must be kept in vehicles that centers use to transport children to and from the center. Evidence: During the inspection on July 15, 2015, there was not documentation in center vehicles regarding emergencies as listed above. Plan of Correction: All needed information was added to the van . The Director will make sure the folder is updated regularly.
