Discussed revisions to CPS request form and fee increase for central registry search.

Comments:
An unannounced monitoring inspection was conducted on 7/14/2015 from 11:00am - 1:35pm. The following staff to child ratios were observed: 2's - 11 x 2 staff 3's - 9 x 1 staff 4's - 7 x 1 staff 4's & 5's - 7 x 1 staff SA - 26 x 2 staff Children were observed during nap. Two diaper changes were observed in the two-year old class. Field trip and transportation procedures were observed in the school age class. Records were reviewed for four children and two staff. Additionally reviewed: Documentation of injuries, medication, the posted menu, documentation of emergency practice drills, and hand washing. Please complete the "plan of correction" for each violation cited on the violation notice and return it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word "corrected" is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Standard #: 22VAC40-185-160-C
Description: Based on record review, the center failed to ensure at least every two years from the date of the first initial TB screening, staff members shall obtain and submit the results of a follow-up tuberculosis screening. Evidence: The most recent TB screening on file for staff #1 was conducted on 6/20/2013.

Plan of Correction: The staff member did have a TB test done on June 10, 2015 and had a negative reading on June 12, 2015. However, paperwork was misplaced. Staff member has repeated test on July 14, 2015 with a negative reading and is now in staff members file.
