Comments:
Today an uannounced renewal inspection was conducted with the Principal-Mrs. Amanada. This visit took place in the morning between 9am-12:30 there 7 classrooms observed with a total of 150 children with 23 staff within the supervision guidelines. The children were observed having outside play time, riding bikes, morning snack, story-time, water play, lunch and nap time. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records,fire drill log, medication, and emergency procedures were observed during this inspection. Violations:
Standard #: 22VAC40-185-160-A
Description: Based on observation of the staff records, one staff record did not include documentation of a TB screening. Evidence: Staff #2 did not have documentation of a TB screening.

Plan of Correction: Principal stated that she will have them complete it this week.

Standard #: 22VAC40-185-240-D-4
Description: Based on observation and a review of staff records, a staff was observed giving medication however the staff member did not have the medication administration training. Evidence: Staff #2 was observed giving medication however the staff member did not have the MAT training conducted by the Virginia board of nursing.

Plan of Correction: Principal stated that she thought the training was universal and stated she will make sure the staff member have the training as soon as possible.

Standard #: 22VAC40-185-270-A
Description: Based on observation of the physical plant, the following areas of the center were found not maintained in a clean, safe and operable condition. Evidence: In the intermediate bathroom, chipped paint and 4 coin size holes were found on the wall.

Plan of Correction: Principal stated that the soap dispenser came off the wall, she will have it put back on the wall.

Standard #: 22VAC40-185-280-B
Description: Based on observation of the classroom, a hazardous substance was found unlocked and accessible to children. Evidence: In the intermediate classroom, a hazardous substance was found in a cubbie unlocked and accessible to children.

Plan of Correction: Principal placed the item in a locked cabinet.

Standard #: 22VAC40-185-280-H
Description: Based on observation of the classroom, a purse was found accessible to children. Evidence: In the school age classroom, a purse was found accessible to children .

Plan of Correction: Principal placed the purse in a locked storage closet.

Standard #: 22VAC40-185-510-D
Description: Based on observation of the medication authorization forms the following were beyond the expiration date. Evidence: 2 medication authorization forms were expired.

Plan of Correction: Principal will return the forms to the parent and will request a new one immediately.

Standard #: 22VAC40-185-560-G
Description: Based on observation of the food brought from home, several did not contain the child's name and date. Evidence: 5 lunch containers did not include the child's name and or date.

Plan of Correction: Principal will remind parents and staff to label all lunch container's brought from home.
