22VAC40-191 Background Checks (22VAC40-191)

An unannounced monitoring inspection was conducted from 10:15 a.m.- 1:00 p.m. with the director. 30 children, ranging from age 4 months to 9 years, were in the care of ten staff. Children were observed napping, having show and share time, working on a name art project, and eating lunch. Lunch served today was grilled chicken breast sandwich, corn, chilled peaches, and milk. The last fire drill was 7/15/15. The last shelter-in-place drill was 6/16/15. Two staff are certified in medication administration training (MAT). 25 staff are certified in first aid and CPR. Eight medications were reviewed. Four children's records were reviewed and in compliance. Three staff records were reviewed. Standard #: 22VAC40-185-70-A
Description: Based on documentation review, not all staff records were complete. Staff A (hired 6/4/15) did not have documentation of two or more reference checks prior to hire.

Plan of Correction: I will have that staff resubmit them.

Standard #: 22VAC40-185-270-A
Description: Based on observation, not all inside areas of the center were maintained in a clean, safe, and operable condition. Evidence: The infants room had batteries in an unlocked drawer, the toddlers room had batteries, Krazy Glue, a pair of scissors, and a screwdriver in unlocked drawers, the two year olds room had a wall air vent with a thick layer of dust, and the toddlers room cold water faucet was not working.

Plan of Correction: The infant room teacher moved the batteries during the inspection. The toddlers room teachers will move the items. The two year olds room will move or cover the sand table near the air vent. The vent will be dusted. We will put in a maintenance request to have the cold water faucet fixed.

Description: Based on observation, cleaning and sanitizing materials were not kept physically separate from food. Evidence: In the school age classroom, liquid dish detergent and sanitizing spray were on the same shelf as packaged doughnuts, crackers, and macaroni and cheese.

Plan of Correction: The school age teacher will remove either the cleaning and sanitizing materials or the food.

Description: Based on observation, not all electrical outlets had protective covers. Evidence: The threes room had two uncovered outlets, and the toddler classroom had two uncovered outlets.

Plan of Correction: The teachers will put the outlet covers back on.

Standard #: 22VAC40-185-500-B
Description: Based on observation and staff interviews, the center failed to clean diapering surfaces with soap and water. Evidence: The infants, toddlers, and twos classrooms all use a quaternary disinfectant spray (EPA registered) as a one step process to both clean and sanitize diapering surfaces. The diapering surface shall be cleaned with soap and water, then sanitized with a bleach water solution or commercial chemical disinfectant, and left to air dry for a minimum of two minutes.

Plan of Correction: We will get bottles of soapy water for these classrooms.

Description: Based on a review of medications, the center did not follow procedures for administering medication. Evidence: Child A's CVS brand ibuprofen expired in April 2015.

Plan of Correction: We will have the parent bring in another and take home the expired medication.
