Technical Assistance:
Discussed medication authorization forms. The LI printed the DSS sample form for the center to consider using.

Comments:
An unannounced monitoring inspection was initiated today at 1:30pm. There were 44 children under the supervision of 6 staff. During the inspection, children were observed taking naps, coloring pictures, completing arithmetic worksheets, and interacting with the staff in a positive and constructive manner. Children have the option of bringing lunch from home or eating what the school provides. Today, the children were offered breaded fish (or macaroni and cheese for vegetarians), green beans, diced pears and milk. The findings of today's inspection were reviewed with and given to the administrative assistant. Once the director completes the Plan and Date of Correction, this is to be mailed to the Fairfax Licensing Office within 14 calendar days. Should you have any questions, please contact me.

Standard #: 22VAC40-185-340-A
Description: Based on file review and observations, the center is unable to ensure the care and protection of all children in care at all times. Evidence 1. Child #6 has a medication authorization form (signed by the physician) for benedryl and an epi-pen while in care at the center effective through 12/31/15. The epi-pen is available for use, but the benedryl was not brought into the center, so the staff are unable to administer this in case of emergency.

Plan of Correction: The staff had been notified by the parent that the benedryl is not needed for the child. They will ask the parent to provide this in writing on the medication authorization form.

Standard #: 22VAC40-191-60-D-1
Description: Based on file review, 2 out of 5 staff records does not have documentation that the criminal history record request, or the request for search of the central registry, was submitted within seven calendar days of the person being employed. Evidence 1. Staff #4's date of hire is 6/22/15. There is no documentation on file that these background checks have been submitted. 2. Staff #5's date of hire is 6/29/15. There is documentation that the search of the central registry was mailed on 7/15/15.

Plan of Correction: Staff #4's form has been received and mailed to Central Registry.
