Comments:
An unannounced monitoring inspection was conducted on 8/4/15 between 4:45 and 6:00. The inspection was conducted with the director. The inspector spoke with staff and children. The following staff/child ratios were observed: There were 36 school age children with two staff. The children were engaged in various activities including playing games. Snack this afternoon was watermelon and mini-muffins.Health inspection ? 6/10/15 Fire inspection ? 7/15/15 Last fire drill 7/17/15 and three shelter-in-place drills have been conducted with the last one on 6/26/15 All required documents were posted including license, licensing notices, menu, and evacuation information.

Violations:
Standard #: 22VAC40-185-130-C
Description: The center failed to obtain verification of the required school immunizations for two children. Updated immunizations are required to be verified between each child's fourth and sixth birthdays (shots required before attending school). Evidence: Four children's records were reviewed and two children did not have verification of their school immunizations. Child #2's (DOB 8/1/08) last immunization verification is dated 8/9/10 when the child was two years old. The child is now seven years old. Child #3's (DOB 3/7/05) last immunization verification is dated 7/15/08 when the child was three years old. The child is now 10 years old. Both children were in care at the pre-school site and the immunizations were obtained prior to their beginning care there.

Plan of Correction: Get records from parents by 8/6/15.
