Comments:
An unannounced monitoring inspection was conducted on 8/4/15 between 3:30 and 4:45. The inspection was conducted with the office assistant and completed with director at the school age site. The inspector spoke with staff and children. The following staff/child ratios were observed: 2?s - one staff with six children 3?s - one staff with six children 4?s - one staff with nine children The children were engaged in various activities including playing with puzzles, story time, and playing a game of tossing rings. Snack this afternoon was watermelon and mini-muffins. The children were also served water with sliced apples in the water. Health inspection ? 6/10/15 Fire inspection ? 7/15/15 Last fire drill 7/31/15 and a shelter-in-place drill was conducted on 5/13/15 All required documents were posted including license, licensing notices, menu, and evacuation information.

Violations:
Standard #: 22VAC40-185-270-A
Description: The center has not maintained the front steps to the center in a safe condition. Nail heads are sticking up on the steps leading to the front door. Evidence: The inspector observed one nail head sticking up approximately one-half an inch. This was seen on the left side of the steps by the hand rail. Several other nails heads are also not flush against the wood of the steps. The nail heads could be a tripping hazard.

Plan of Correction: Will be fixed on 8/5/15.

Standard #: 22VAC40-185-320-A
Description: The center has not ensured that the restroom's water temperature does not exceed 120 degrees. Evidence: The inspector used a thermometer to test the water temperature in the hall restroom. The water tested at 125.1 degrees. The maximum water temperature is 120 degrees for the sinks used by the children.

Plan of Correction: Water will be fixed on 8/5/15.
