22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted from 12:00 p.m. to 2:45 p.m. At the time of entrance the following adult to child ratios were observed: 10 school aged children, between the ages of 5 and 12 years old with 1 staff 20 children, four years old, with 1 staff (nap time) 12 children 2 1/2 to 3 1/2 years old with 1 staff (nap time) 9 children, 23 months to 2 1/2 years old, with 2 staff 4 infants, 10 to 15 months old, with 1 staff The records sample size consisted of 4 children?s records and 4 staff records. Children were observed resting quietly at nap time, playing board games and watching a children's movie, being held and bottle fed (infants) and playing with floor toys. Medication administration was not observed. However, a sampling of 4 medications were reviewed in terms of storage, labeling, written authorizations and required documentation. Standard #: 22VAC40-185-60-A Description: Based on a review of records, the licensee did not ensure that a separate records containing all of the required documentation is available for each enrolled child. Evidence: Four children's records were reviewed. The record for child #4 did not contain documentation of the center viewing proof of the child's identity and age. Plan of Correction: The the owner and administrator (staff #5) stated that she has viewed the child's birth certificate at the time of emrollment in June 2011, but has misplaced the documentation completed at the time of viewing it. She states that she will contact the child's parent and ensure that she views the birth certificate and documents having done so in the child's record by no later than Monday, August 17th.
