Discussion was held on the topics of current enrollment and current staffing. Discussion of a search of the central registry for Staff #3. An error was made with regards of a request being returned. The registry asked for the center (by phone) to mail back this request. An e-mail was sent to the center from the registry confirming that a search conducted on the date of inspection was "clear" for Staff #3.
The following inspection took place between the hours of 9:30 am through 1 pm. Today, I met with the Staff of the Chesterbrook Academy #822 located in Fair Lakes, Virginia. At the start of this unannounced renewal inspection there were a total of 67 children with 11 staff; good ratios in all 6 rooms. A total of 4 children?s files and 3 new staff files were reviewed. The children were engaged in the following: story time, diaper changing, hand washing, diapering, art, centers within the classroom, large motor skills on the playground, lunch, nap time, and other organized activities. Standard #: 22VAC40-185-60-A Description: Based on review, 2 of 4 children's files reviewed indicated that not all of the required documentation was contained within each child's record. Evidence - 1. On the date of inspection (09/02/2015), the Proof of Identification for Child #2 was not available for review. The start date listed for Child #2 was 03/15/2013. 2. On the date of inspection, the Health Information forms (consisting of the child's physical and immunization page) for Child #1 was not available for review. The start date for Child #1 was listed as 03/18/2013. Plan of Correction: The applicable parents will be contacted and all missing documentation will be placed on file.
Standard #: 22VAC40-185-70-A Description: Based on review, 3 of 3 staff files reviewed indicated that not all of the required information was on file. Evidence - 1. On the date of inspection (09/02/2015), no references were on file for Staff #1, Staff #2, and Staff #3. At least two references for each staff person is required. 2. On the date of inspection, the emergency contact information for Staff #3 was not available for review. Plan of Correction: All missing information shall be obtained and placed in the applicable staff file.
Standard #: 22VAC40-185-270-A Description: Based on observation, areas and equipment of the center, inside and outside, were not maintained in a safe and operable condition. Evidence - 1. On the date of inspection (09/02/2015), the blue toddler chair located in the Beginner's Room was frayed in several spots. 2. On the date of inspection, the playground swing was missing 1 of 4 swings. The missing swing seat's chain was tied around the top of the swing set. In addition, the chains appear heavily rusted near the top of each of the three remaining swings. 3. On the date of inspection, the outdoor wood bench located under a canopy of the playground shows signs of raised wood on it's seat. The area could produce splinters or possibly cut a child if sat upon or slid into from the side. Plan of Correction: All areas will be addressed. The blue chair will be replaced. Maintenance will be contacted to address and repair the swings, and a private contractor has already been contacted to repair the outside bench.
Standard #: 22VAC40-185-500-B Description: Based on observation and interview, the diapering surface was not used only for diapering or cleaning children. Evidence - 1. On the date of inspection (09/02/2015), the diapering surface within the Beginners Room was being used to store new supplies and a change of clothes for children making the transition into this classroom. 2. The Lead teacher stated (during an interview conducted on the date of inspection) that the clothes and supplies were not normally kept there, but was only temporary until bend assignments (on the above shelving) were complete. Plan of Correction: All supplies and other items will be relocated from this area.
