Technical Assistance:
For safety reasons, it is highly recommended that the exit doors from the toddler classroom and the 2's classroom be replaced. The toddler door does not stay open, which presents the possibility of it closing when children are transitioning to and from the classroom to the playground. The 2's door does not open properly and gets stuck in the corner, which presents a safety hazard in the event of an emergency evacuation. During this inspection, the Inspector was not able to open the 2's door without the assistance of a classroom staff person.

Comments:
An unannounced inspection was conducted today. The Inspector was on site after receiving information from center management personnel regarding a supervision incident on August 31, 2015. One violation was cited regarding this incident. Additional violations were cited as a result of the on-site monitoring inspection conducted today. There were 63 children in care, with a total of 10 staff members assigned to their direct supervision. The children were observed creating art projects, building with blocks, playing with creative toys, coloring pictures, playing with playdoh, and practicing letter recognition skills. This inspection was completed between approximately 8:45 am and 11:00 am. Thank you for meeting with me today. Standard #: 22VAC40-185-70-A
Description: Based on a review of new staff records, it was determined that one record was not complete. Evidence: Staff record #3 does not include emergency contact information for the staff person.

Plan of Correction: I will remind her to complete the form today.

Standard #: 22VAC40-185-270-A
Description: Based on observations, it was determined that two items in the center have not been maintained in a safe and operable condition. Evidence: 1. One of the toy cars on the 2's playground is missing the steering wheel and is no longer operable as designed. 2. On the back wall of the building, a piece of plywood has been installed to cover a water valve opening in the bricks. The piece of plywood has deteriorated and is now splintered and cracked, with much of the wood peeled off. The piece of plywood is at the level of the school-age children.

Plan of Correction: 1. I will throw away the play car. 2. I will ask maintenance to replace the piece of wood on the brick wall.

Standard #: 22VAC40-185-280-B
Description: Based on observations, it was determined that one spray bottle of cleaning solution was not locked. Evidence: In the 2's classroom, the door to the cabinet beneath the changing table was not locked. The spray bottle of sanitizing solution was stored inside the unlocked cabinet.

Plan of Correction: The cabinet was locked immediately during the inspection.

Standard #: 22VAC40-185-340-F
Description: Based on staff interviews, it was determined that on August 31, 2015 one child was left beyond the sight and sound supervision of any staff members for a brief period of time. Evidence: It was reported to the Inspector that on August 31, 2015 a child in the toddler classroom was left beyond the sight and sound supervision of any staff members for approximately 1 1/2 minutes. At approximately 4:00 pm, there were 10 children in care in the toddler classroom with a total of 2 staff members. During the transition from the classroom to the outdoor playground, one toddler remained inside after the doors were closed and the other children and both staff members were outside. The staff members did not realize the child was alone in the classroom until approximately 1 1/2 minutes after they were outside.

Plan of Correction: The teachers did a head count as the children were walking down the stairs to enter the playground. When conducting the head count teacher A immediately realized that one child was inside and proceeded to return to the classroom and retrieve the child. The child was alone for less than 2 minutes. Since this incident, we have put new transition procedures into place. Each time the toddler class transitions from the classroom to the playground, one of the staff in the classroom radios to the office and an extra staff person is sent to the classroom to assist while the children exit the classroom. In addition, when the toddler class returns to the classroom an extra staff person is sent to assist while the children exit the playground. One staff person is always stationed at the front of the line, one staff person is always stationed at the back of the line, and one staff person is always standing at the door to hold it open during these transition times.

Description: Based on a review of new staff records, it was determined that one record was not complete. Evidence: Staff record #3 does not include a signed Sworn Disclosure Statement (SDS.) This person's date of hire was 6/3/15.

Plan of Correction: I will make sure she signs the form today.
