Comments:
An unannounced complaint investigation was conducted on August 21, 2015 and today due to a complaint received in the Valley Licensing Office on August 19, 2015. There was not a preponderance of evidence to support the allegation that there was missing medication and a lack of supervision of a child in care. The complaint is determined to be not valid. Any violations found not complaint related are identified on the violation notice. While at the facility the licensing inspector participated in a shelter-in-place practice that went quite smoothly. Description: Based on documentation and interview the facility failed one day to administer medication as prescribed to a child in care. Evidence: 1. On 08/21/2015 a review of documentation indicated child #1 missed a prescribed medication that had been provided the facility by the parent. 2. On 08/21/2015 staff #1 stated child #1 one day in July did miss the administration of a medication due to a communication issue encountered. 3. On 09/24/2015 documentation on the Infant Daily Report for child #1 indicated the child did not get a medication on July 16, 2015 as prescribed.

Plan of Correction: The medication box will be kept in the office. When prescriptions come in there will be a photograph taken of the bottle or packaging for the record. We are also reviewing our policy as we may make a change with regards to incoming medications to include a more thorough checklist signed by the parent and facility as to the contents of any medication containers.
