Comments:
The inspector arrived at 1:30 PM and departed at 3:45 PM. Transition from nap time, afternoon snack service and activities were observed. A sampling of five children's records and five staff records were reviewed. Medication administration and injury records were reviewed. An exit meeting was conducted with the program director during which the facility's Plan of Correction was obtained. Please contact the inspector if there are any questions.

Standard #: 22VAC40-185-70-A
Description: Based upon review of 5 staff records, the facility has not ensured that each staff record includes documentation that two or more references as to character and reputation as well as competency were checked before employment. Evidence: There was no documentation in the records provided that references were obtained prior to the hire of staff 3 or staff 5, both hired 8/11/2015.

Plan of Correction: The reference checks were completed on 9/23/2015. However, future staffing decisions will not be made until reference checks have been completed.
