Consultation was given on additional staff trained in Daily Health Observation to ensure all hours of operation are covered if a staff is absent. Training opportunities are listed on the VA DSS web site under Professional Training and Development.
An unannounced monitoring visit was conducted. All rooms were in ratio with direct care staff. Observed children in circle time, transition to hand washing and am snack, a yoga class and transition to the outside play area. Reviewed staff records, medication and child record, emergency preparation, care of children and nutrition. Staff records are very organized and current based on Director's spreadsheet. Areas of non-compliance are identified in this report. Please sign and date the report and return it to me within 10 calendar days from date of receipt. Standard #: 22VAC40-185-40-J Description: Based on record review, an annual injury prevention plan was not completed for 2015. Evidence: The last annual review of center injuries was dated July 2014 and the Director stated that one has not been completed since that date. Plan of Correction: Update injury prevention plan.
Standard #: 22VAC40-185-270-A Description: Based on observation, the center has 2 areas that are not being maintained in a clean or safe manner. Evidence: 1. Ceiling tiles are missing in the two upstaires rooms around the air vents. The vents also need to be cleaned as dirt and dust is visible. 2. A small red chair in the 3's classroom is torn in several small places. Plan of Correction: Will fix tear in chair or replace completely. Will speak to Church about replacing tiles.
Standard #: 22VAC40-185-280-B Description: Based on observation, a cleaning product was not maintained in a locked place. Evidence: A bottle of 409 cleaner was stored in the upstairs hall cabinet which was not locked during the inspection. Plan of Correction: 409 was moved to locked cabinet.
