22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted from 10:00 a.m. to 1:00 p.m. At the time of entrance the following adult to child ratios were observed: 8 infants, ages 9 to 15 months, with 2 staff 9 toddlers, 15 to 24 months old, with 3 staff 21 two year old children with 3 staff 7 three year old children with 2 staff 7 four year old children with 1 staff The records sample size consisted of 6 children?s records and 6 staff records. Children were observed engaged in free play with toys and manipulatives, creating pumpkin art, building with blocks at the table, participating in circle time with discussion about letters and sounds, watching a children's television program, being diapered and bottle fed, eating lunch which consisted of grilled cheese sandwiches, peaches, green beans, and milk. Hand washing, sanitizing and rest room procedures were observed. Medication administration was not observed. However, a sampling of ointments, insect repellent and sunscreen were reviewed with regard to storage, labeling, written authorization and required documentation. Areas of non-compliance are identified on the violation notice. Thank you for your time during today's inspection. Please do not hesitate to contact me if I may be of assistance. The licensee provided verbal responses to the violations during the inspection, which were entered on to the violation notice by the Licensing inspector.
Standard #: 22VAC40-185-160-A Description: Based on a review of records, the licensee did not ensure that each staff member submits documentation of a negative tuberculosis screening no later than 21 days after employment which was completed within 12 months prior to or 21 days after employing or volunteering. Evidence: Six staff records were reviewed. The record for staff #4 who began employment on 4/13/15 contained documentation of a Tuberculosis screening which was dated 12/7/13. This screening was conducted more than 12 months prior to beginning employment at the center. Plan of Correction: The owner (Staff #7) stated that this was an oversight and that a new Tuberculosis screening will be conducted today and resuts filed in staff #4's record upon receipt. In the future, the dates will be monitored more closely to ensure compliance with the required time frames.
