Consultation today included room arrangement to decrease distractions during transition times.
An joint investigation was initiated on 8/19/15 following a self-reported concern regarding supervision issues. The following standards were part of the inspection. 340 A: When staff are supervising children, they shall always ensure their care, protection and guidance. 340 F: Children under 10 years of age always shall be within actual sight and sound supervision of staff, except when toileting. After careful consideration, there was a preponderance of evidence to support the concern about supervision, and both standards were found to be in violation. While not part of the direct concern, another violation was discovered during the course of the investigation. 270 A: Areas and equipment of the center shall be maintained in a safe and operational condition. Standard #: 22VAC40-185-270-A Description: During today's inspection, some areas of the center were observed not to be maintained in a operable condition. Evidence: The door exiting from the first classroom (Classroom # 1) did not latch securely when closed; and the metal gate closure on the outdoor fence to the playground did not provide a secure closure as intended today. Standard #: 22VAC40-185-340-A Description: When staff were supervising children on 8/12/15, they did not ensure their care, protection and guidance of a 4 year old child when that child was found on his own in the playground. Evidence: When staff were supervising Child # 1 on 8/12/15, staff were unaware that he was out on the playground by himself. Child #1, (who attended special needs classes part time this summer) was able to either leave the building on his own or was left outside when Class # 2 returned to their room. According to reports, on that afternoon at approximately 4:15 pm, Child # 1 and Child # 2 were moved from Class #1 to Class #2. Documentation of this, which is part of the program's "transition box" policy, was not complete for that time. The class left the building to go to the playground. They returned at approximately 5:00 pm. Staff report that name-to-face attendance was not done according to normal procedure. However, two staff recall seeing Child # 1 indoors. At approximately 5:20 pm, Staff # 3 from the school-aged class went outdoors to find a child's missing object. While there, she found Child # 1 playing with mulch. There was not a preponderance of evidence to support if the child was left on the playground or exited the classroom on his own. Plan of Correction: Not available online. Standard #: 22VAC40-185-340-F Description: Through staff interviews and record review, it was determined that a child (age 4) was not being supervised within sight and sound supervision when he was found alone on the playground. Evidence: According to staff interviews and record review, Child # 1 was unaccounted for approximately 10 - 20 minutes on 8/12/2015. The child was found playing by himself on the playground by Staff # 3 at approximately 5:20 pm. There has not been a preponderance of evidence to prove if Child # 1 did not enter the school with Class #2 when the class returned from the playground; or if he exited from the building on his own after the class returned to their room. Standard #: 22VAC40-185-350-F Description: While children may be temporarily reassigned to another class for reasons of administrative necessity, staff report that this has been a regular occurrence after 4:00 pm this summer. Evidence: Staff report and documentation confirms that children have been brought to other classes as numbers drop. On 8/12/15, Staff # 1 reported bringing Child # 1 and Child # 2 to Staff # 2 to keep numbers in ratio. Plan of Correction: Not available online. Contact Inspector for more information.
