Comments:
The inspector arrived at 11:45 AM and departed at 2:15 PM. Lunch services, activities and transition to nap time were observed. A sampling of five children's records and four staff records were reviewed. No children's injuries were reported this school year. Medication administration was reviewed. An exit meeting was conducted with the program director during which the facility's Plans of Correction were obtained prior to the conclusion of the inspection. Please contact the inspector if there are any questions.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based upon review of five children's records, the facility has not obtained documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Evidence: The facility was unable to produce documentation that child 3 and child 5 received the required immunizations. Both children began attending the facility on 9/8/2015.

Plan of Correction: The facility responded with the following: Health Coordinator will contact parents for shot records A.S.A.P.

Standard #: 22VAC40-185-60-A
Description: Based on review of five children's records, the facility has not ensured that each child's record includes the addresses of at least two designated persons to be contacted in an emergency if the parents cannot be reached. Evidence: 1. The record for child 2 does not include an address for one emergency contact person. 2. The record for child 4 does not include the city and state or zip code for one emergency contact person.

Plan of Correction: The facility responded with the following: I will get the family service workers to parents to come in and complete address on permission slip form A.S.A.P.
