The licensing inspector conducted an unannounced monitoring inspection Monday, September 21 from 10:22am to 12:30pm. The children were observed playing with toys, working in small groups and singing songs in large group time. The infants were observed in exersaucers interacting with their teacher. Three children were observed participating in an extracurricular dance class. Lunch consisted of bologna sandwiches, cucumbers with ranch dressing, apple slices, chips and milk. Interviews were held with staff throughout the inspection. Additionally, when appropriate the inspector interacted with children in each classroom. All classrooms and playgrounds were inspected today as well as the facility?s mini buses. Three children?s records and four employee records were reviewed during this inspection. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 8/11/2015 Shelter-in-place drills: 1/20/2015 and 7/15/2015 Last fire inspection: 6/11/2015 Last health inspection: 5/27/2015 Total enrollment: 110 Today, the following child to staff ratios were observed: Infants ?4:1 Older Infants-3:1 Toddlers ?4:1 Twos ? 9:3 Threes ?12:2 Pre-K ?7:1 School Age ?not present The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-270-A Description: Based on observation on 09/21/2015, the center did not ensure that all areas of the center, inside and outside were maintained in a safe condition. EVIDENCE: The fence on the playground has a slat which is broken at the top and has a jagged, sharp edge. Areas and equipment of the center, inside and outside, shall be maintained in a safe condition. Unsafe conditions shall include, but not be limited to, visible cracks, breakage of any equipment; and other components that could entangle clothing or snag skin. Standard #: 22VAC40-185-280-B Description: Based on observation on 09/21/2015, the center did not ensure that hazardous substances such as cleaning products are kept in a locked place that prevents access by children. EVIDENCE: 1. A bottle of cleaning solution was observed in an unlocked cabinet in the Pre-K classroom. 2. The inspector observed a canister of disinfectant spray in the unlocked bathroom in the Two's room. Standard #: 22VAC40-185-330-B Description: Based on observation on 09/21/2015, the center did not ensure that the required amount of resilient surfacing was present on the playground. Evidence: 1. The depth samples taken on the school age and twos playgrounds measured less than 6 inches deep. The school age playground has slide/climbing equipment. The two's playground has two rocking spring toys. Where playground equipment is provided, resilient surfacing shall comply with minimum safety standards as described in the American Society for Testing and Materials standard and shall be under equipment with moving parts or climbing apparatus. Standard #: 22VAC40-185-430-C Description: Based on observation on 09/21/2015, the center did not ensure that the s-hooks on play equipment used by the children are not open more than the thickness of a penny. EVIDENCE: The s-hooks on the swings on the school-age playground and two swings on the preschool playground have an opening greater than the thickness of a penny. S-hooks on play equipment used by the children shall have openings that may not be open more than the thickness of a penny. Plan of Correction: Not available online.
