An unannounced monitoring inspection was conducted today, 9/29/15. Upon arrival to the school the children were having snack. Following snack they went into the gym and played and then split into smaller groups to begin homework. There are 25 children ranging in age from 4-10 years with three staff. A review of children's records was also conducted. A review of the violations was conducted during the inspection. An acknowledgment of inspection conducted & licensee responsibility was left with the center. The violation notice and inspection summary was e-mailed to the facility on 9/30/15 for a plan of correction. Should you have any questions please do not hesitate to call. Inspection began: 3:10 pm and ended approximately 4:20 pm.

Standard #: 22VAC40-185-60-A
Description: Based on review of children's records the center failed to obtain all the required information. Evidence: 1. Child record #2 did not have complete addresses of the emergency contacts listed.

Plan of Correction: Parents will be contacted and ask to provide complete addresses. (sic)

Standard #: 22VAC40-185-280-B
Description: Based on observation during the inspection, the center did not have hazardous substances locked. Evidence: 1. The spray Lysol can was stored in a pocket on the outside of the first aid backpack. 2. This backpack was located on the floor next to the door and tables used by the children.

Plan of Correction: Once Lysol is used to clean tables, staff members will lock said cleaner in storage cabinet. (sic)

Standard #: 22VAC40-80-120-E-2
Description: Based on observation during the inspection the most recent violation notice was not posted. Evidence: 1. The most recent violation notice was in a notebook that contained the children's records. 2. This was not available to the public.

Plan of Correction: Recent violation notice will be posted on board. (sic)
