1. Discussed gymnastics being an activity of potential high-risk, and the need for close supervision.

Conducted an investigation into allegations of a lack of supervision during outdoor play time and gymnastics. Interviews conducted and observations made. There was insufficient to support a finding of a lack of supervision during outdoor play time however, there was a preponderance of evidence to support a finding that there has been a lack of supervision during gymnastics/tumbling. This complaint is deemed to be valid/founded.

Description: Based on interviews conducted and observations made, it was determined that staff were not supervising children in a way that ensured their care, protection, and guidance during gymnastic/tumbling activities. Evidence: 1. Staff stated that the typical supervision during gymnastics/tumbling activities would include: Staff #1 would be sitting near the exit door to open the door and greet parents, while Staff #2 would be roving all activities in the gym and taking children to the bathroom located down the hallway. 2. Staff have not been directly supervising or spotting the gymnastics/tumbling activity, and would not be located directly at the gym mats where this activity was occurring. Staff have not maintained a close proximity to this activity, or remained close enough to assist with the tumbling or prevent unwanted falls. 3. Staff stated that they were not trained in gymnastics/tumbling and have not been providing instruction. This is one of many activities offered during free-play in the gym on a daily basis, and is initiated by the children. 4. On 9/17/15, when the inspector arrived at the center, Staff #1 was sitting by the exit door, and Staff #2 was sitting on the stage. Multiple simultaneous active games were taking place in the gym, with the gymnastics mats located about 30'-40' from where stay were located. The inspector observed children doing hand stands and cart wheels with no direct staff supervision at the mats. One child was observed attempting a hand stand and falling onto his head and shoulders.

Plan of Correction: Staff will directly monitor + help with approved gymnastic activities. Students will not be allowed to do flips, headstands, handstands, jumps or any other activity that could result in harm.
