Technical Assistance:
Discussed with Director & Parent the requirements regarding the Virginia Health Forms.

Comments:
Today an unannounced monitoring visit took place in the morning with Director-Mrs. Saida between 11:40-2pm. Six classrooms were observed with a total of 108 students with 17 staff within the supervision guidelines. The children were observed having morning outdoor free play, learning about geometrical shapes and sizes this week, lunch, napt time and afternoon dismissal. Positive interactions were observed during this inspection. A complete inspection of the children and staff records, fire drill log, medication, interview with a parent, emergency plan, accident/incident reports and emergency pocedures were observed during this inspection. Standard #: 22VAC40-185-450-A
Description: Based on observation of the cots, two cots were found without a top and or bottom cover. Evidence: In the yellow & purple classroom, two cots were found missing a top cover.

Plan of Correction: Director stated that they will remind parents to provide a top cover and or will provide one for the children.
