Comments:
An unannounced monitoring inspection was conducted on 10/7/15 from 10:16am - 12:10pm. During the inspection there were 67 children ages 4 months to 12 years old in care with 13 staff. A tour of the facility was conducted and children were observed in a variety of activities in the classrooms including arts and crafts, writing, playing with blocks, sensory activities and story time. Children were also observed playing outside and eating lunch. Records were reviewed for five children and five staff. Medication, emergency procedures, and emergency supplies were reviewed. Areas of non-compliance are identified on the violation notice, and were reviewed during the exit interview. lease complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Standard #: 22VAC40-191-60-C-1
Description: Based on a review of five staff records, it was determined that the facility did not deny continued employment of a staff who did not have a criminal history record report within 30 days of employment. Evidence: 1. The record for staff #5 (date of hire 8/24/15) did not have documentation of a completed criminal history record report. Staff #5 was present during inspection and working with children in the orange classroom. 2. Staff #6 (Program Director) confirmed that she had not received the criminal history record report for staff #5.

Plan of Correction: The Program Director will follow up with the Virginia State Police to check the status of the criminal history record check for staff #5 and document the conversation. If necessary, a new criminal history record check will be completed for staff #5. For all new employees, if their criminal history check is not received within 30 days, a call will be placed to the Virginia State Police to check status and the conversation will be documented.
