Technical Assistance:
Technical assistance today included documentation of incident/injury reports.

Comments:
An unannounced monitoring inspection took place today from 1:30 - 2 :15 pm. There were 12 children present with 3 staff. I observed children playing ping pong, foosball and a board game. The new unit focus is on Brazil, and children were using some Portuguese words. The two new classrooms were also measured. Standard #: 22VAC40-185-550-M
Description: During a review of incident/injury reports at the center today, some were not completed as required. Evidence: Of three reports reviewed today, there was only one signature on two reports from 9/16/15; and there was no documentation of the time and method that the parent was notified in one report on 9/16.

Plan of Correction: We will make sure to complete the reports and get two signatures.
