63.2 Facilities and Programs..

Comments:
The licensing inspector conducted an unannounced monitoring inspection from 9:35am - 12:15pm. There were fourteen preschool children in care with two staff. The children were observed at circle time, playing in the centers, at story time and preparing for lunch. Three children's records were reviewed. Areas of non-compliance are identified on the violation notice. If you have any questions, please contact your licensing inspector at (276) 608-3758. Thank you for your cooperation.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, the center failed to maintain the playground in a safe condition. Evidence: 1. The inspector observed the playground. There are two pieces of wood left from the removal of the large swing structure. The wood is sticking out of the pea gravels and is cracked and splintered with sharp edges.

Plan of Correction: The teacher will complete a request for maintenance and ask that the remaining wood from the swing structure be removed. Staff at the center will cover the wood pieces with pea gravels and monitor the children closely in this area until maintenance is complete. Staff will monitor the playground for unsafe conditions.

Standard #: 22VAC40-185-330-B
Description: Based on observation and measurement, the center failed to have 6 inches of resilient surfacing on the playground. Evidence: 1. The inspector observed the playground and measured the resilient surfacing (pea gravels). Pea gravels measured approximately 2 to 3 inches in the fall zones on each side of the platform on the fort. Fall height of the equipment is 42 inches.

Plan of Correction: The teacher will complete a request for maintenance and ask that additional pea gravels be placed in these areas. Staff will monitor the resilient surfacing on the playground to ensure it remains at the required height.
