Technical Assistance:
Discussed long term authorization forms for medication.

An unannounced inspection was conducted today from 9 am until 1015. There were 75 children with 12 staff and all groups were engaged in various activities. Some children were observed coming inside from outside play, some observed hand washing, bathroom time, and others were engaged in table activities. The Inspector observed all three floors and reviewed supervision. Staff files, emergency procedures, attendance, accident reports, and evacuation drills were also review. Description: Based on record review and staff discussion, the annual fire permit is not current. Evidence: 1. Staff stated there have been coordinated inspections and working through the process to receive a current inspection permit. 2. Permit that is posted on the wall has an expiration date of August, 2015.

Plan of Correction: We will continue to work with the local fire department to get the inspection permit current.

Description: Based on observation, there was an unsafe situation in which a light was plugged into the wall and the lamp was on the other side of a sink. This had the electrical cord in close proximity to a water source and was observed in a children's bathroom.

Plan of Correction: This was corrected immediately during the inspection by unplugging the light and removing the cord.

Description: Based on observation, there was a Vapor Rub medication in a classroom's unlocked cabinet.

Plan of Correction: This was removed during the inspection.

Standard #: 22VAC40-185-510-L
Description: Based on staff interviews and record review, staff did not record administering medication to a child. Evidence: 1. A medication consent form had a start date of October 2 through October 8. The consent was for a prescription medication to be given at 12 each day. 2. The staff stated the medication has been given but that there was not a record of administering the medication.

Plan of Correction: A discussion will be conducted with staff to ensure all administered medication is recorded.

Description: Based on record review and staff interviews, the center has not conducted monthly practice evacuation drills. Evidence: 1. Drill records do not have record for drills in September, July, February, and March of this year.

Plan of Correction: We will conduct monthly drills.

Standard #: 22VAC40-191-40-D-1-C
Description: Based on record review of two new staff files, the center has not obtained a completed central registry background search for one staff. Evidence: 1. Staff #2's employment is recorded as August 17, 2015 and background search is not in the file. 2. Staff stated the forms were sent in August and that they will call to determine status.

Plan of Correction: We will continue to monitor timely return of background checks.
