22VAC40-191 Background Checks (22VAC40-191)
An unannounced inspection was conducted from approximately 10:15 a.m. until 12:30 p.m. Upon entering the center, there were 14 children in care ranging from thirteen months old to three years old with four staff supervising at all times. An additional staff arrived during lunch. Children were observed singing Halloween songs to a video on the t.v., free play, playing outside, washing hands, eating lunch, and preparing for nap time. Three children's records were reviewed and three staff records were also reviewed. The violations from the previous inspection had been corrected. Standard #: 22VAC40-185-60-A Description: Based on record reviews, the center failed to obtain all required information for the children's records. Evidence: 1. The inspector reviewed three children's files. One of three records (record # 2) did not have information for allergies and intolerance to food, medication, or other substances, and actions to take in an emergency. Record #3 did not have an address for one of the emergency contacts. Plan of Correction: The director will get the information from the parents. Director will review all files to make sure all files are complete.
Standard #: 22VAC40-185-70-A Description: Based on record review, there was not a staff person on duty with current CPR certification. Evidence: 1. Three staff records were reviewed. The inspector and director reviewed the copies of the first aid and cpr card. The CPR card states that the CPR was given 4/26/2014 and was valid for one year after completion date, therefore having expired 4/26/2015. Plan of Correction: The director will contact the administer of the course to seek clarification, and if it is valid for 2 years, a new card will be given. If not, the director will arrange to complete the course.
Standard #: 22VAC40-185-270-A Description: Based on inspection, the facility did not maintain the center in a clean condition. Evidence: 1. The inspector observed the changing pad with a ripped place in it. Plan of Correction: The director stated she would replace the mat.
Standard #: 22VAC40-185-280-H Description: Based on observation, the center did not keep medications and other harmful agents in an area inaccessible to children. Evidence: 1. The inspector observed baby powder, butt balm, and neosporin located by the sink in the room used to change diapers. Plan of Correction: Director corrected during the inspection. Director will remind staff that items need to be locked, and she will check throughout the day to make sure items are locked.
Standard #: 22VAC40-185-320-B Description: Based on observation and measurement, water measured over 120 degrees. Evidence: 1. The inspector used the thermometer to meaure the temperature in the bathroom off the kitchen that the children use and it measured 124 degrees. Plan of Correction: The director will have the water heater adjusted so that it does not go over 120 degrees. Director and staff will supervise children closely until the temperature is adjusted to the correct degree.
Standard #: 22VAC40-185-440-L Description: Based on observation, there was a pillow in the crib and a child under two was in the crib. Evidence: 1. The inspector observed a staff person supervising a child being ut to sleep in a crib and there was a boppy pillow in the crib. Plan of Correction: The staff person removed the pillow during the inspection. The director will review with staff that pillows, toys, etc. are not to be in a crib.
Standard #: 22VAC40-185-560-M Description: Based on observation, staff did not sit with children during meal times. Evidence: 1. The inspector observed lunch time and did not see staff sitting with the children. Plan of Correction: The director and staff will start sitting with children during lunch.
Standard #: 63.2-1701-F Description: Based on observation the center did not have posted required documents. Evidence: 1. The director stated the license had fallen from the wall. 2. The most recent inspection was not posted. Plan of Correction: The director will find the license and hang it back up , and will post the most recent inspection. Director will check the license and inspection periodically to make sure they are still posted.
