Description: Based on observation, 3 children's information was posted and not kept confidential without written parent permission. Evidence: Three children's allergies were posted near the food prep area and the center director conformed that written permission from the parents had not been obtained prior to posting the information.

Description: Based on observation, the diaper pad was not sanitized after use. Evidence: The Licensing Inspector observed a staff member completing a diaper change and the pad was not sanitized. The center uses paper to cover the pad and the staff member stated that she did not realize it must also be sanitized between uses.

Plan of Correction: Sanitizing steps were taken immediately.

Description: Based on record review, one of ten injury forms was incomplete. Evidence: The staff member did not complete the "Future Action To Prevent Reoccurrence"
