Comments:
Thank you for your time and assistance with the unannounced monitoring inspection conducted on October 22, 2015. The on-site visit was from 10:55 a.m. to 3:30 p.m. On this date, at the time of arrival, there were twenty-seven (27) children in two classrooms and in the care of five staff members. Two additional staff members were available to assist, if needed. The total enrollment for the program was sixty-four (64) children. The exception to compliance was noted on the Violation Notice. The risk rating for the violation was listed on the Supplemental Page. The violations from the previous inspection were reviewed for compliance. There were nine staff members' records reviewed or updated and four children's records reviewed. There were five staff members and three children interviewed during the visit. The medication and medication paperwork were reviewed for two children. Lunch was observed to meet the USDA guidelines and consisted of grilled cheese sandwiches, chicken noodle soup, lima beans, fruit cocktail, and milk. First aid and emergency supplies, the playground, water temperature, and required posted information were reviewed. Activities observed included: outdoor play, fall art activities, activities involving nocturnal animals, story time, transitions, diapering procedures, hand washing and bathroom procedures, lunch, nap time and dismissal procedures. Standard #: 22VAC40-185-260-A
Description: Based on review of documentation and verification with the director, the center failed to obtain an updated annual fire inspection. Evidence: 1. During review of records on 10/22/2015, it was determined that a fire inspection had not been completed within twelve months of the most recent inspection. The date of the most recent fire inspection was 06/03/2014.

Plan of Correction: The fire marshal was called during the visit and a fire inspection was requested. The fire inspection was completed and the report obtained on 10/28/2015. The director is now aware that they must request an annual fire inspection.
