Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records, it was determined that two records were not complete. Evidence: 1. Child record #3 does not include "authorized pick ups" information. 2. Child record #4 does not include: the father's name/number/address; the phone number for emergency contact person #2.

Plan of Correction: The parents will be asked for the missing information.

Standard #: 22VAC40-185-350-E-5
Description: Based on observations, it was determined that the program operated beyond the allowable staff:child ratio of 1:18 for approximately 15 minutes. Evidence: Upon arrival, there were 42 children in care with a total of 2 staff members. Two additional staff members arrived approximately 15 minutes after the start of this inspection. For that 15 minute period of time, the program was operating beyond the maximum allowable ratio of 1 staff for every 18 children.

Plan of Correction: There are additional kids here today because it is a school closure teacher workday. On days like this, we will stagger our staffing pattern better to ensure that staff:child ratios are met at all times.

Standard #: 22VAC40-185-510-C
Description: Based on a review of children's medications, it was determined that three medications have expired. Evidence: 1. Based on the prescription label directions, one child's refrigerated medication was to be discarded on 10/19/15. 2. Two long term prescription medications expired on 10/31/14.

Plan of Correction: Both parents will be asked for new medications.
