63.2 Facilities and Programs..
An unannounced inspection was conducted today with other local agencies in response to an incident reported by the center director. Based on interviews conducted and documentation and recordings reviewed, a violation was cited for staff to child ratios for infants and is identified in the Violation Notice. Standard #: 22VAC40-185-350-E-1 Description: Based on staff written statement and an audio recording of staff interviews, for children from birth to the age of 16 months, the center did not ensure that there was one staff member for every four children. Evidence: 1. On October 15, 2015 there was one staff member supervising six infants alone during the morning hours in the Infant 2 classroom. Staff B staff stated that Staff D was alone with a total of six children. When Staff C arrived to the classroom, there was a total of seven children supervised by two staff, which exceeds the 1:4 teacher/infant ratio. 2. Staff D confirmed that she was alone with six children on October 15, 2015 and at 8:15am when Staff C arrived, there was a total on seven infants. 3. Staff B reported that "on Friday" two staff were supervising a total of nine children, which exceeds the 1:4 teacher/infant ratio. Plan of Correction: Staff schedules have been changed to accomadate [sic] children arriving earlier.
