Comments:
An unannounced monitoring inspection was conducted today, 10/26/15. Violations were reviewed and an Acknowledgement of Inspection Conducted & Licensee Responsibility was signed and left at the facility. Upon arrival to the center the LI first went to the main office and reviewed staff records and then went over to the center. The children were involved in free play time while waiting on the remaining children to arrive from the various schools served by this center. There were 51 children in care with five staff. Snack was served and hand washing was observed. Should you have any questions please do not hesitate to call. Inspection began: 1:30 pm and ended 4:15 pm.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation of the center during the inspection it was found that there was some areas that needed repair. Evidence: 1. Located in the art room, the wall painted black had large pieces of sheetrock peeling off. These areas are accessible to the children.

Plan of Correction: Piece that was hanging was taken off and there is no visible hanging pieces present. [sic]

Standard #: 22VAC40-191-40-D-1-C
Description: Based on review of staff records the facility failed to complete repeat background checks on staff. Evidence: 1. Staff #2 has a criminal history check dated 8/23/12; and a central registry check dated 9/10/12. 2. Both of these record checks have expired.

Plan of Correction: We will submit a background check for this employee on 11/13/15. [sic]

Standard #: 22VAC40-191-60-C-1
Description: Based on review of staff records the center failed to obtain background checks within the 30 day time frame of hire. Evidence: 1. Staff #3 was hired 9/15/15 and the criminal history check was not mailed off until 10/16/15. 2. Staff #3 continues to work at the center.

Plan of Correction: mailed on 10/16/2015. [sic]

Standard #: 22VAC40-191-60-C-2
Description: Based on review of staff records the center failed to obtain background checks within the 30 day time frame of hire. Evidence: 1. Staff #3 was hired 9/15/15 and the central registry check was not mailed off until 10/16/15.

Plan of Correction: Mailed on 10/16/2015. [sic]

Standard #: 22VAC40-191-60-D-1
Description: Based on review of staff records it was found that the facility failed to send off background checks within seven calendar days. Evidence: 1. Staff # 3 was hired 9/15/15 and the central registry check and the criminal history check was not mailed off until 10/16/15.

Plan of Correction: Mailed on 10/16/2015. [sic]

Standard #: 22VAC40-80-120-A-6
Description: Based on observation during the inspection it was found that the center was operating outside of their terms on the license. Evidence: 1. During the inspection it was found that the center was providing care for children four years of age. The centers license allows the youngest to be five years of age.

Plan of Correction: Child was removed from our program. [sic]
