Comments:
An unannounced monitoring inspection was conducted beginning at 1:25 p.m. and ending at 3:45 p.m. There were twenty-eight children in care ranging in age from seven months to four years old with six staff. Infants were observed sleeping, being fed and in having play time in an exersaucer. Toddlers, two year olds, three year olds and four year olds were observed having naptime. Preschool children were observed preparing for snack, handwashing and having snack. Snack today was vanilla pudding and juice. After preschool children had snack, toddler and two year olds had snack. Snack today for the toddlers was applesauce and milk. Classrooms were observed. The playground was not inspected due to weather. Three staff records and five children's records were reviewed. Standard #: 22VAC40-185-160-C
Description: Based on review of documentation, the center failed to obtain updated Tuberculosis (TB) test or screening every two years as required. Evidence: Staff C's last documented TB test was dated 12/4/2012.

Plan of Correction: The staff has scheduled to have the TB updated.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center failed to ensure hazardous substances were kept in a locked place. Evidence: In the bathroom, there was a can of Glade air freshener located on the windowsill and one can of Glade air freshener located in an unlocked cabinet above the left sink.

Plan of Correction: The air fresheners were moved to a locked place during the inspection.

Standard #: 22VAC40-185-520-C
Description: Based on observation, the center failed to obtain written authorization for diaper ointment and failed to label the container with the child's name. Evidence: A tube of desitin diaper ointment was not labeled and there was no documentation of authorization from the parent.

Plan of Correction: The ointment was labeled during the inspection. A new authorization will be requested from the parent.
