An unannounced renewal inspection was conducted today at 8:05am. There were 28 school-agers directly supervised by 4 staff. The physical plant, 4 staff training records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. A self-reported incident was also investigated during this visit. There was an abundant supply of books, toys, and materials for the children. Children were observed participating in group play, reading books, playing dance games, and eating donuts. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. If you have any questions regarding this inspection, please contact the Licensing Inspector. Keesha Minor (email@example.com) (703)479-4680
Standard #: 22VAC40-185-340-F Description: Based on a self reported incident, the facility failed to ensure that children under 10 years of age are always within actual sight and sound supervision of staff. Evidence: While the class was outside on the playground, 2 children left the playground unbeknownst to the staff and waled into the building and to the SACC room, where a third staff member was. There were 2 staff members on the playground that did not see the children leave the playground, and the staff member inside of the classroom contacted the outdoor staff to let them know that the 2 children had come inside unsupervised. Plan of Correction: Outside staff are to position themselves in pre arranged areas of the playground which allows for all exit points to be visible. Entrance door #7 is checked daily by the roving teacher to assure that it is locked.
