Comments:
The inspector conducted unannounced monitoring inspection from 11:20 am until 1:15 p.m. There were 10 children in care with staff supervising at all times. Children were observed making a craft, watching a video, playing, washing hands for lunch, eating lunch, and on the bus. Three children's records were reviewed. Violations from the last inspection had been corrected, there was one repeat violation. Violations are noted on the inspection summary. Violations:
Standard #: 22VAC40-185-130-A
Description: Based on record review, the center did not obtain immunizations before a child attended the center. Evidence: 1. The inspector reviewed three children's records. One of three records (child record #1) did not have an immunization in the record. The dates of enrollment was 8/17/2015.

Plan of Correction: The center will get the immunization record. Files will be reviewed to make sure they are complete.

Standard #: 22VAC40-185-140-A
Description: Based on record review, the center did not have a physical examination within 30 days in a child's record. Evidence: 1. The inspector reviewed three records. Two of three children's records did not have physical examinations (child record #1 and #3).

Plan of Correction: Physical examinations will be received from the family.

Standard #: 22VAC40-185-60-A
Description: Based on record review, the center did not have the required information in a child's record. Evidence: 1. The inspector reviewed three records. One of three records (record #1) did not have proof of ID.

Plan of Correction: The director will get proof of ID from the family. Files will be reviewed to make sure all information is complete.

Standard #: 22VAC40-185-330-B
Description: Based on observation and measurement, resilient surfacing on the playground was not the required depth. Evidence: 1. The inspector measured the mulch and it was between 2-3 inches around the red slide, the double red slide, and the yellow climber.

Plan of Correction: The playground will not be used until the required depth of 6 inches is on the playground.

Standard #: 22VAC40-185-560-F-4
Description: Based on observation, potential choking hazards were offered to children three years of age or younger. Evidence: 1. The inspector observed hotdogs being served for lunch. They were cut in half, but not in smaller pieces as required.

Plan of Correction: The inspector will send the TA. Teachers will cut hotdogs based on the TA for children 3 and under.
